Reconsideration (Re-eval) Process

When you are requesting a reconsideration or re-evaluation of a bill that has been previously processed, you will need to submit a copy of the bill, the EOR and any documentation to support the charges and mail to:

You can also fax your request to 406-495-5020. Reconsiderations/Re-evals may take up to 30 days to process.
