Critical Access Hospitals and Type of Bill 85X

Type of Bill (TOB) 851-857 indicate services that are provided in a Critical Access Hospital (CAH). Montana State Fund requires all CAH’s billing for facility services on a UB04 to use TOB 85X when submitting bills for payment. Professional services provided by a CAH still need to be billed on a CMS 1500 per the Administrative Rule of Montana 24.29.1433(9).
