Conscious Sedation – What You Need to Document

99143, 99144 and 99145 states ”provided by the same physician performing the diagnostic or therapeutic service that the sedation supports, requiring the presence of an independent trained observer to assist in the monitoring of the patient’s level of consciousness and physiological status; age 5 years or older” so remember that your documentation needs to include information regarding the “independent” trained observer in order to support payment of these codes. The observation cannot be done by the practitioner providing the diagnostic or therapeutic service as they cannot effectively do both.
